Our community cataract portal allows patients to be seen for their post-operative assessment in the community, rather than in the hospital, whilst maintaining a complete hospital patient record.
mediSIGHT EMR automatically generates a letter for patients to take to their preferred optometrist following their cataract operation. The letter contains a web link and unique PIN code that allows the optometrist to access the portal securely without handling patient identifiable data. When the patient goes for a post-operative assessment, the optometrist simply enters the PIN into the online portal. This gives them access to an online post-cataract assessment, where they enter information about the condition of the patient’s vision and eye health, which transmits back into the hospital patient record.
Our general community portal works in the same way as the community cataract portal but can be used for other shared care schemes such as Glaucoma and AMD.
Patients are provided with a letter and PIN number to provide to the Community Clinician and data is returned directly back to the mediSIGHT patient record.
