All requests for custom audits must be submitted using the form below. This ensures that the required details are provided for security checks and to deliver accurate and complete audits.

Purpose of Custom Audit *
Please detail the purpose of this audit. Please also indicate if this is an FOI request or for submission to a national reporting programme and any response timescales.

Hospital / Organisation name (to be used for the custom audit) *

Please define what information each column should return if this is not immediately apparent from the column name.

A blank Excel file containing only column headers can be uploaded as an example. Please DO NOT include any patient identifiable data, including hospital or NHS numbers, as this is not a secure form.

Selection criteria *
Please provide all inclusion/exclusion criteria that you wish to use. e.g. hospital names to include or exclude, specific diagnoses to include or exclude, active diagnoses to include or exclude, procedures to include or exclude, consultants to include or exclude etc.

Audit date range *
Please provide the required start and end date and the date type to be used for this range e.g. visit date, procedure date, diagnosis date etc.

Audit granularity required / expected *
For example, 1 row per patient, 1 row per eye, 1 row per procedure etc.

Additional instructions / notes
Please provide any further information that you feel may be useful e.g. defining acronyms, VA notation required (logMAR, ETDRS letter score, Snellen metres etc.). If you have requested a similar audit before then please provide details (including any previous ticket numbers) so that it can be updated and re-used.

Our estimated turnaround for a simple custom audit request is 2 weeks. This is dependent on a timely agreement of the detailed requirements for each audit.

Medisoft Custom Audit Service Definitions

The Audit Suite must be installed and accessible to authorised Medisoft users to fulfil these requests. Data extracts will only be provided via secure means to authorised personnel at the organisation responsible for the data.

Custom Audit Service (simple)

This is a unique service included in the support fees for Medisoft EMR customers. Requests are limited to one-off data extracts or simple additions to the custom audit launcher in the Audit Suite, and a limited but fair usage policy is applied. Requests must be raised via this webpage. All requests will then be assessed to ensure complexity is within the scope limits for this service. Any requests that can be fulfilled by using the Audit Suite will not be accepted and instead guidance will be provided on how to access this data using the Audit Suite.

Custom Audit Service (complex)

A chargeable service is available to Medisoft EMR customers for complex data extracts or complex additions to the custom audit launcher in the Audit Suite. Requests must be raised via this webpage. All requests will then be assessed to establish complexity and development timescales.
