Glaucoma Shared-Care

The system has been designed to meet the agenda of moving care of stable glaucoma / ocular hypertension patients to the community optometrist setting whilst maintaining supervision by the Hospital Eye Service (HES). This monitoring by the HES is made possible by Medisoft’s ‘Virtual glaucoma review’ screen.

Community optometrists can link in a secure fashion to the Hospital server, and specific nurse and optometrist glaucoma assessment screens have been created to assist with the Glaucoma Shared-Care pathway.
